health care professionals, experienced in caring for patients who need
rehabilitation, works with both the patient and family to develop a plan of
care that meets each individual’s needs. Our goal is to enable individuals

physical therapists, occupational therapists, speech pathologists,
respiratory therapists, recreation and activity specialists, rehabilitation
technicians and case managers/social workers.

assess and evaluate each patient admitted to the unit and participate in
developing the overall plan of care. Nursing staff work closely with the
patient, physician and family to assure that the patient’s needs and the
family’s concerns are fully addressed.
-
Occupational Therapy— Occupational Therapists help patients regain
independence with self care and daily tasks and motivate people to
effectively cope with the limitations imposed by their illness. Based on
the patient’s needs, activities may focus on dressing, grooming and
hygiene tasks, strengthening the upper body, developing fine motor skills,
compensating for visual impairments or memory loss as well as sensory and
problem solving.
- Physical
Therapy--- The physical therapist facilitates the patient’s recovery
by helping the patient to improve strength, relieve pain and re-learn
activities such as standing, walking or stair climbing. Patients
participate in therapeutic exercises to strengthen muscles, provide
balance training or movement re-education or functional training to enable
the patient to stand or walk.
- Speech and
Language Therapy—In addition to evaluating patients with swallowing or
communication disorders, the speech pathologist works with patients who
have difficulty understanding or producing language, or who need
assistance in producing proper speech sounds.
- Case
Manager--- The case manager works closely with the treatment team, the
patient and family to develop an appropriate plan for the patient after
discharge. The case manager assists patients who are discharged to their
homes with referrals to outpatient rehabilitation facilities, home care,
etc. For patients who require alternative discharge plans, the case
manager coordinates referrals to long term rehabilitation facilities,
skilled nursing facilities, nursing homes, etc

muscle weakness or other respiratory problems and provides appropriate
treatment as prescribed by the patient’s physician. The respiratory
