Care Management/Social Services: 

The Care Management/Social Services Department at St. Joseph’s Wayne Hospital consists of Masters-prepared social workers, nurse case managers, and home care coordinators.   They provide support during a patient’s stay in the hospital and upon discharge. Recommendations may include home care, hospice, medical equipment, nursing home placement, rehabilitation referrals, drug and alcohol counseling or counseling to help patients and their families adjust to illness or injury.
